Request Forms

We would ask that if you generate your own request forms out of your practice management system (such as Medtech), could you please make Aotea Pathology the main provider, and correct the opening hours where relevant. Click here for an instruction page to help with this procedure.

Test request forms designed specifically for specialist practitioners working in private practice, private clinics and hospitals are available. These forms include fields/tick boxes for the essential information required to process these tests requests, and for which charges normally apply. Some exemptions may be applicable and for information about the DHB's policy on charges/exemptions for specialist referred tests. Click here

Aotea Pathology operates a network of 18 collection rooms through out the greater Wellington region. Our rooms at Courtenay Place in central Wellington are open from 7am each weekday, as are the rooms at Raroa Road in Lower Hutt. Patients may attend any of the collection rooms in the Wellington, Lower Hutt, Upper Hutt, Porirua and Kapiti areas. For laboratory details and opening hours of all rooms please. Click here

A range of specimen collection supplies is supplied by the laboratory. Please use your Laboratory Supplies requisition form and fax it through to (04) 381 5980, or send it in to the laboratory with the courier in the usual way. Alternatively email stores@apath.co.nz with your order.

Access to results

We wish to reassure all referrers that historical laboratory results are available for patients regardless of the laboratory they have attended in the past. Please phone the results line (04) 381 5901.

Turnaround time

Our aim is to maintain the current prompt turnaround times including urgent testing where there is clinical need. To achieve this we have planned our combined collection and courier system to work as efficiently as possible to ensure that specimens are delivered promptly to the main testing laboratory at all times of the day. For example sample sorting and data entry is undertaken in our main hub rooms at Porirua, Raroa Road and Courtenay Place. This allows ready sample identification and tracking throughout the process. Couriers leave collections rooms and main hubs on a regular basis to take specimens to the Courtenay Place laboratory.

We closely monitor turn around times to ensure that service standards for urgent and routine testing as prescribed in our contract with the DHB are met or bettered. If you have any concerns or comments please let us know. This is helpful for us to identify problems and address them promptly.
